Return these forms with your application.

All adults who will be living in the assisted housing unit must sign, date and include their social security number on the application. Faxed copies will not be accepted.

NOTE: All address changes and other updates to your application must be submitted to Lake MHA in writing within 10 days of the change. Lake MHA does not forward mail or accept any changes over the phone.

Parking Vehicle Registration

Residents who own a vehicle must have the vehicle registered in their name and must obtain a Lake MHA parking sticker. All visitors must park in authorized visitor parking.

Community Service

Each month, every non-exempt adult resident must contribute eight hours of community service or participate in an economic self-sufficiency program for eight hours. There are volunteer opportunities at many local organizations throughout Lake County. Please complete your monthly community service time sheet.

Housing Choice Voucher (HCV) Forms

THE HOUSING CHOICE VOUCHER PROGRAM WAITING LIST IS NOW CLOSED. WE WILL NOT BE ACCEPTING ANY HCV APPLICATIONS UNTIL THE WAITING LIST RE-OPENS.

  • You may now check our website to see if you made the waiting list. Please visit: “Waiting List Status & Position Look-up” (click here).  Enter your social security number, if your information does not appear, you did not make this HCV waiting list.
  • Please do not call the office to check on the status of your application. The office staff will not take your social security number over the phone.
  • It is important that you notify our office in writing if your address changes. Application update form (click here). We will send you a letter when your application is pulled.

Homeownership Program

Applications may be mailed the Lake MHA Office: 189 First St., Painesville, OH 44077 or faxed to (440) 354-5008. You must be a current voucher program participant in order to apply for the Homeownership Program.

Family Self-Sufficiency (FSS) Program

FSS is a voluntary program to help HCV participants become financially self-sufficient through employment. You must be a current voucher program participant in order to apply for the FSS Program.

Forms for Landlords

Information Required from Landlord

  • The Electronic Funds Transfer (EFT/Direct Deposit) form must be completed along with a voided check and returned to our office.  EFT/Direct Deposit form
  • A W-9 form must be completed and returned to our office. The form may be emailed, faxed or mailed to our office.
  • Proof of ownership is required for all HCV properties.

Change of Address Form

A Landlord Change of Address Form must be completed and returned to our office in order to update changes. This form must be signed, dated and may be submitted to Lake MHA by mail, fax or email.

Rent Increase Form

Please make any request for annual rent increases in writing by returning this form at lease sixty (60) days prior to the effective date of the requested rent increase.

Inspection Checklist

Please make sure to review this checklist and to correct any deficiencies prior to the scheduled HQS inspection. The unit MUST be in MOVE-IN condition PRIOR to the inspection date and all work must be completed at this time.
