Information Required from Landlord

  • The Electronic Funds Transfer (EFT/Direct Deposit) form must be completed along with a voided check and returned to our office.  EFT/Direct Deposit form

  • W-9 form must be completed and returned to our office. The form may be emailed, faxed or mailed to our office.

  • Proof of ownership is required for all HCV properties.

If your rental property has been rented, please contact our office to have your rental removed from the Landlord List.

Change of Address Form

A Landlord Change of Address Form must be completed and returned to our office in order to update changes. This form must be signed, dated and may be submitted to Lake MHA by mail, fax or email.

Please complete the Landlord Property Form to be listed on the Lake MHA Housing List and/or on our website.

Rent Increase Form

Please make any request for annual rent increases in writing by returning this form at lease sixty (60) days prior to the effective date of the requested rent increase. Please note, if the tenant signature is not available, you must include a copy of the proper rent increase notice provided to the tenant, 60 days prior.

Inspection Checklist

Please make sure to review this checklist and to correct any deficiencies prior to the scheduled HQS inspection. The unit MUST be in MOVE-IN condition PRIOR to the inspection date and all work must be completed at this time.
