All adults who will be living in the assisted housing unit must sign, date and include their social security number on the application. Faxed copies will not be accepted.
NOTE: All address changes and other updates to your application must be submitted to Lake MHA in writing within 10 days of the change. Lake MHA does not forward mail or accept any changes over the phone.
Lake MHA does not provide emergency housing assistance – see information on see information on the Partner page.
Residents who own a vehicle must have the vehicle registered in their name and must obtain a Lake MHA parking sticker. All visitors must park in authorized visitor parking.
Each month, every non-exempt adult resident must contribute eight hours of community service or participate in an economic self-sufficiency program for eight hours. There are volunteer opportunities at many local organizations throughout Lake County. Please complete your monthly community service time sheet.
This program (formerly known as Section 8) offers low-income households an opportunity to lease privately owned rental housing at an affordable rate.

FSS is a voluntary program to help HCV participants become financially self-sufficient through employment. You must be a current voucher program participant in order to apply for the FSS Program. For more information contact: Victoria Smith – 440-354-3347, extension 19.
Helping Families with Affordable Housing
A Landlord Change of Address Form must be completed and returned to our office in order to update changes. This form must be signed, dated and may be submitted to Lake MHA by mail, fax or email.

Please complete the Landlord Property Form to be listed on the Lake MHA Housing List and/or on our website.
Please make any request for annual rent increases in writing by returning this form at lease sixty (60) days prior to the effective date of the requested rent increase.

Please make sure to review this checklist and to correct any deficiencies prior to the scheduled HQS inspection. The unit MUST be in MOVE-IN condition PRIOR to the inspection date and all work must be completed at this time.
