Applicants, residents, and other interested parties may submit written comments regarding the proposed Multifamily RAD PBRA House Rules, including the Smoke Free Lease Addendum to Lake MHA in person, by mail, or by emailing Victoria Smith, vsmith@lakemetrohousing.org or Theresa Lee, tlee@lakemetrohousing.org
Written comments must be received by 5:30 P.M. on April 18, 2024. The proposed rules and addendum may be viewed on Lake MHA’s website (www.lakehousing.org) or in Lake MHA’s Administrative Office. The proposed rules/addendum are subject to change.

HUD (92006) form – Supplement to Application Federally Assisted Housing
All adults who will be living in the assisted housing unit must sign, date and include their social security number on the application. Faxed copies will not be accepted.
NOTE: All address changes and other updates to your application must be submitted to Lake MHA in writing within 10 days of the change. Lake MHA does not forward mail or accept any changes over the phone.

Parking Vehicle Registration
Residents who own a vehicle must have the vehicle registered in their name and must obtain a Lake MHA parking sticker. All visitors must park in authorized visitor parking.
